1. INTRODUCTION

Welcome to Dentopass.

This Privacy Policy explains how Dentopass LLC (“Dentopass,” “the Company,” “we,” “us,” or “our”) collects, uses, shares, stores, and protects information provided through our international dental patient coordination services. By using Dentopass services, submitting information through our forms, or otherwise interacting with us, you acknowledge that you have read and understood this Privacy Policy.

2. WHO WE ARE

Dentopass is a patient coordination service that helps international patients connect with independently owned and operated dental clinics in Mexico (“Partner Clinics”).

Dentopass is not a dental clinic, healthcare provider, medical practice, insurance company, or travel agency. Dentopass does not provide dental treatment, medical advice, diagnoses, second opinions, or healthcare services of any kind. All clinical decisions, diagnoses, and treatment recommendations are made exclusively by the treating Partner Clinic and its licensed dental professionals, following an in-person clinical examination.

Regarding HIPAA. Dentopass is not a healthcare provider, health plan, or healthcare clearinghouse, and does not transmit health information in connection with insurance claims or billing transactions. Based on these facts, Dentopass does not consider itself a “covered entity” under the U.S. Health Insurance Portability and Accountability Act (HIPAA). Regardless of this classification, Dentopass takes reasonable, good-faith measures to protect health-related information consistent with the principles described in this Policy.


3. INFORMATION WE COLLECT

Dentopass does not collect personal information directly through its website. All information is voluntarily submitted by users through secure third-party forms provided by Fillout (“Fillout Forms”). Dentopass collects two general categories of information: information submitted by patients, and information submitted by Partner Clinics.

3.1 Information Submitted by Patients

Contact Information

  • First name and last name
  • Email address
  • Phone number
  • Country of residence
  • City or region

Treatment Information

  • Dental concerns
  • Treatment interests
  • Treatment preferences
  • Travel availability
  • Budget considerations

Health-Related Information

  • Dental radiographs (X-rays)
  • Oral photographs
  • Relevant medical history
  • Treatment-related information voluntarily provided by the patient

Satisfaction Surveys
Following treatment, patients may voluntarily provide reviews, testimonials, ratings, feedback, and photographs.

3.2 Information Submitted by Partner Clinics

Dental clinics that apply to join, or that participate in, the Dentopass network voluntarily submit information through Fillout Forms, which may include:

  • Clinic name, address, and contact information
  • Names, credentials, and specialties of dental professionals
  • Photographs of clinic facilities (reception area, treatment rooms, sterilization area, laboratory, imaging room)
  • Services offered and signature procedures
  • Accepted payment methods
  • Licensing and accreditation information provided by the clinic

Dentopass independently reviews publicly available information (such as online reputation, credentials, and patient reviews) about prospective Partner Clinics before extending an invitation to join the network. This independent review process is separate from, and in addition to, the information a clinic submits directly to Dentopass.


4. HOW INFORMATION IS COLLECTED

Information is collected through:

  • Fillout Forms
  • Email communications
  • Telephone communications
  • SMS communications
  • Satisfaction survey forms

Dentopass does not operate patient portals, user accounts, or online dashboards.


5. HOW WE USE INFORMATION

Information is used solely for purposes related to patient and clinic coordination services. These purposes include:

  • Connecting patients with Partner Clinics
  • Facilitating communication between patients, Patient Coordinators, and Partner Clinics
  • Preparing preliminary treatment estimates and clinic profiles
  • Coordinating appointments and travel logistics
  • Assisting with treatment inquiries
  • Providing ongoing coordination support before, during, and after treatment
  • Conducting follow-up communications and collecting satisfaction surveys
  • Maintaining administrative, contractual, and financial records related to Partner Clinics and Patient Coordinators
  • Improving coordination services

Dentopass does not sell personal information. Dentopass does not use patient information for advertising or marketing campaigns. Dentopass does not send newsletters or unsolicited promotional communications.


6. INFORMATION SHARING

Dentopass may share information only with the categories of recipients described below. In the preceding twelve (12) months, Dentopass has disclosed personal information to the following categories of recipients for the business purposes described in this Policy:

Partner Clinics (Category: Service Providers)
Information may be shared with Partner Clinics participating in the Dentopass network for purposes including treatment evaluations, treatment estimates, appointment coordination, and patient communication. Prior to a patient’s in-person arrival at a clinic, patient information is shared using an internal reference code rather than the patient’s full name, consistent with Dentopass’s internal privacy procedures.

Patient Coordinators (Category: Independent Contractors)
Patient Coordinators are independent contractors, not employees of Dentopass. They may access information as necessary to provide coordination services and communicate with patients and Partner Clinics, and are contractually bound to confidentiality and data-handling obligations.

Service Providers (Category: Technology Vendors)
Dentopass uses Fillout as its form collection and data management platform, described further in Section 8.

Legal and Regulatory Authorities (Category: Legal Compliance)
Dentopass may disclose information if required by applicable law, court order, government authority, tax authority, or other legal process.

Dentopass does not sell, rent, or trade personal information to third parties.


7. AUTHORIZATION TO SHARE INFORMATION

By submitting information through Dentopass, you authorize Dentopass to share your information, including health-related information, radiographs, photographs, treatment inquiries, and other submitted materials, with Partner Clinics and Patient Coordinators as necessary to provide coordination services. You acknowledge that this sharing is necessary for Dentopass to perform its coordination functions, and that Dentopass has no control over the clinical use of this information once received by a Partner Clinic acting in its independent professional capacity.


8. THIRD-PARTY SERVICE PROVIDERS

Dentopass utilizes Fillout as its primary form collection and data management platform. Information submitted through Dentopass forms is collected and stored through Fillout. Dentopass does not maintain proprietary databases or servers containing patient or clinic information.

Fillout operates under its own terms, privacy practices, and security measures, independent of Dentopass. Users are encouraged to review Fillout’s privacy documentation separately.

If Dentopass adopts additional third-party service providers in the future (including but not limited to payment processing, banking, or communication platforms), this Policy will be updated to reflect those relationships.


9. DATA RETENTION

General Information. Contact and treatment-related information submitted through Fillout is retained for up to one (1) year from the date of submission. After this period, such information is permanently deleted from the Fillout system in accordance with Dentopass’s data retention practices, unless a longer retention period is required under Section 9.1 below.

9.1 Financial and Administrative Records. Notwithstanding the general retention period above, Dentopass retains financial, contractual, tax-related, and administrative records including but not limited to commission records, invoices, and Organizer/Beneficial Owner documentation for the period required under applicable U.S. federal tax law and Wyoming state law, which may extend up to seven (7) years or longer where required.

9.2 Legal Holds. Dentopass may retain information beyond the periods described above where required by applicable law, in connection with a legal dispute, or for other legitimate business purposes.


10. PARTNER CLINIC RESPONSIBILITY

Once information has been shared with a Partner Clinic, that clinic becomes independently responsible for its own collection, storage, retention, protection, and use of such information under its own policies and applicable laws, including any laws governing the confidentiality of medical records in Mexico.

Dentopass is not responsible for the privacy practices, security procedures, or data retention policies of Partner Clinics. Patients are encouraged to review the privacy practices of any clinic they choose to engage with directly.


11. TESTIMONIALS AND REVIEWS

Patients may voluntarily provide reviews, testimonials, ratings, photographs, and feedback. Only content for which appropriate consent has been provided may be published by Dentopass.

Published content may appear on:

  • The Dentopass website
  • Dentopass’s official social media accounts (including Instagram, Facebook, and X)
  • Marketing materials
  • Promotional content

Published information may include:

  • First name
  • Country or location
  • Review text
  • Rating
  • Voluntarily submitted photographs

Dentopass will never intentionally publish:

  • Full medical records
  • Email addresses
  • Phone numbers
  • Other sensitive personal information not relevant to the testimonial

Users may request removal of published testimonials, reviews, or photographs by contacting: support@dentopass.com


12. INTERNATIONAL USERS AND CROSS-BORDER TRANSFERS

Dentopass serves international patients from various countries. By using Dentopass services, you understand that information may be processed, stored, and transferred across international borders, including between the United States and Mexico, and may be accessed by Dentopass personnel, Patient Coordinators, and Partner Clinics located in Mexico.

Where applicable, Dentopass seeks to follow internationally recognized privacy principles regarding transparency, security, and responsible handling of personal information, regardless of the jurisdiction in which the information is processed.


13. GDPR RIGHTS (USERS IN THE EUROPEAN ECONOMIC AREA AND UNITED KINGDOM)

Where applicable under the General Data Protection Regulation (GDPR) or the UK GDPR, users may have the following rights:

  • Right of access
  • Right of correction (rectification)
  • Right of deletion (erasure)
  • Right to restrict processing
  • Right to object to processing
  • Right to data portability

Dentopass will respond to verified requests within the timeframes required by applicable law. Certain requests may be limited by legal, contractual, or operational requirements, including the need to retain financial records as described in Section 9.1.


14. CCPA/CPRA RIGHTS (CALIFORNIA RESIDENTS)

Where applicable under the California Consumer Privacy Act (CCPA), as amended by the California Privacy Rights Act (CPRA), California residents may have the following rights:

  • Right to know what personal information is collected, used, and disclosed
  • Right to request deletion of personal information
  • Right to request correction of inaccurate personal information
  • Right to opt out of the sale or sharing of personal information (Dentopass does not sell or share personal information for cross-context behavioral advertising)
  • Right to limit the use of sensitive personal information
  • Right to non-discrimination for exercising any of these rights

The categories of personal information disclosed to service providers and contractors in the preceding twelve (12) months are described in Section 6 of this Policy.

15. CHILDREN AND MINORS

Dentopass services are intended primarily for adults.

Individuals under the age of eighteen (18) may only use Dentopass services, or have information submitted on their behalf, with the direct involvement, review, and consent of a parent, legal guardian, or authorized representative, who must submit and manage all related communications and forms.

Dentopass does not knowingly collect information directly from minors without such involvement. If Dentopass becomes aware that information has been submitted by a minor without appropriate parental or guardian consent, Dentopass will take reasonable steps to delete that information.


16. SECURITY

Dentopass takes reasonable measures to protect information from unauthorized access, disclosure, alteration, or misuse, including reliance on the security measures maintained by Fillout as described in Section 8.

However, no system, platform, transmission method, or storage solution can guarantee absolute security. Dentopass cannot guarantee the security of information transmitted electronically, and disclaims liability for unauthorized access resulting from circumstances beyond its reasonable control.


17. COOKIES AND ANALYTICS

As of the Effective Date of this Policy, Dentopass does not use:

  • Google Analytics
  • Meta Pixel
  • Hotjar
  • Microsoft Clarity
  • Advertising tracking technologies

If such technologies are introduced in the future, this Privacy Policy will be updated accordingly prior to their implementation, and, where required by applicable law, appropriate consent mechanisms will be put in place before such technologies are activated.


18. CHANGES TO THIS PRIVACY POLICY

Dentopass may modify this Privacy Policy at any time. Changes become effective upon posting the updated version on the website. The “Last Updated” date at the top of this Policy will indicate when revisions were made. Continued use of Dentopass services after changes are posted constitutes acceptance of the revised Privacy Policy. Dentopass will review and, where necessary, update this Policy at least once every twelve (12) months.

20. ACKNOWLEDGMENT

By using Dentopass services, you acknowledge that:

  • You have read and understood this Privacy Policy.
  • You understand how your information may be collected, used, and shared, including with Partner Clinics and Patient Coordinators.
  • You authorize Dentopass to share information with Partner Clinics and Patient Coordinators as necessary for coordination purposes.
  • You understand that Partner Clinics are independently responsible for information once it has been shared with them.
  • You understand that general contact and treatment information submitted through Fillout may be retained for up to one (1) year, while certain financial and administrative records may be retained longer as required by law.
  • You agree to the practices described in this Privacy Policy.
